The Relapse Risk Score (RRS) system provides clinically-informed risk assessment for adaptive interventions.
Relapse Risk Score (RRS)
The RRS is a 0-100 score calculated from multiple data sources. Higher scores indicate greater relapse risk.
Risk Tiers

Risk Tier Definitions
R0 - Stable (0-20)
Low risk indicators. Beneficiary demonstrates consistent engagement, stable mood, minimal cravings, and strong protective factors.
R1 - Precaution (20-40)
Some precursor indicators present. May include occasional stress elevation or minor craving reports. Warrants increased attention.
R2 - Elevated (40-60)
Active warning signs detected. Combination of elevated stress, increased cravings, or trigger exposure. Targeted intervention recommended.
R3 - High (60-80)
Multiple high-risk indicators present. May include persistent cravings, mood decline, sleep disruption, or reduced engagement. Prompt clinical contact required.
R4 - Critical (80-100)
Immediate risk indicators. May include slip report, crisis-level cravings, or severe mood decline. Emergency support activation recommended.
Risk Trend Display
Show 7-day and 30-day RRS trends:
- Trend line graph - Visual risk trajectory
- Average score - Mean for selected period
- Peak score - Highest point with date
- Direction indicator - Improving, stable, or declining
